Monthly Fee is Per Person (must live in same household):
Ages 0-18 $50 per child with adult membership
Ages 19+ $100 per person if 2 or more adult memberships
Single Member Rates
Monthly Fee is Per Person:
Ages 0-18 $75
Ages 19-50 $110
Ages 51+ $125
Discounts:
10% off if paid annually
5% off if paid semi-annually

Access to a comprehensive patient portal that includes
Self scheduling of appointments
Full visit summaries
Self reporting of health data including vaccines, medications, pharmacy, allergies, and pre visit questionnaires
